Vaccinations and medical insurance.. (Almaty)

Hi, I want to get vaccinated for tick-borne encephalitis because I hear it can be quite a common thing if you go up into the mountains here regularly.
My Kazakh friends seem not to trust the quality of the doctors here and I was wondering if anyone can make any recommendations of where I should go?
On a related subject I’m planning to stay here for a couple of years, I’m currently covered by my travel insurance but before that expires should I be looking into arranging some sort of private medical insurance here? What do most expats do and does anyone have any recommendations for this?
