Single Mother - No objection letter father (Doha)

I am a single mother looking to relocate to Qatar later this year, my company are establishing an office in Doha. I require a no objection letter from the father of my child, I have been in touch with a solicitor about same, but it would be helpful to have an English language example for her to work with, can anyone help?
