Marriage health check for foreigner (Istanbul)

Hello everybody, we are having some difficulties me and my fiancé to find a family health centre for a marriage medical report, actually they have been asking for an ID number and since I am not Turkish I cannot deliver that...so anyone have experienced that before and what can be done to be able to have the medical report without delivering an ID num? Thanks
