Opening a bank account in Kyiv

Hello everyone! Can anyone tell me what documents and process I need to go through to open a bank account in Kyiv? I'm a Canadian Citizen, living in Canada but visit Ukriane regularly (2 or 3 times a year). I have researched a bit online, I understand that I need a Taxpayer Code Number, and my passport to open the account. My main questions concern the Taxpayer Code Number. Do I need a permanent mailing address in Ukraine in order to apply or receive this? Do I need a Ukrainian residential address to open the bank account? Any input on this would be greatly appreciated!!! Thanks in advance for your time and advice :)
