Importing my dog into Lagos from the United States

Hello, I am being stationed in Lagos, and would like to import my dog into the country from the United States. Does anybody have experience doing this? If so, could you help me understand the process?

My initial research tells me you need a rabies vaccination, and a certificate of good health from veterinarian. I am looking for forms, etc to ensure all paperwork has been completed/submitted before traveling so there are no snags at immigration.
