Cardiologists in Leipzig

I am considering moving to Leipzig within the next year or so. I have one problem though. I have Atrial Fibrillation, and would need to have a cardiologist that I could regularly visit. I now take medication to control my Afib, and would need to have my meds available and monitored.
Does anyone know, how one would go about finding one in Leipzig?
