Doctor Recommendations (San José)

I am coming to Costa Rica for some dental work.
I would like some referrals for doctors near CIMA hospital in Escazu for the following:
Dermatologist, Rheumatology, Orthopaedics, (I may need back/neck surgery again), fibromyalgia, etc. I'm hoping to establish a network of professionals where I can return about once a year for my medical needs.
Health care is so expensive in the U.S.
I haven't found much information on this topic elsewhere. The US Embassy there puts out a list, but I wanted some personal recommendations.
