Guide For Expats to do the Physical Exam In SH (Shanghai)

- Log on the official website Protected content , select the English Vision on the top page of right corner. Then Register your account first...
- Click the "introduction of service" on the memu bar, select the one according to your need, then make the appintment through the link on the page.
* PS To get the Registration Form of Temporary Residence from the police station where your residence is located.
-- Should go with the landlord with the property liscence of the apartment
-- Bring your Passport & photocopy of passport
Reminder before go for the examination:
- Passport & photocopy
- Two-inch of head photo (photo shooting service is provided inside)
- Download the form when you fill out for reservation & print it out
- If applying for working,applicants should also brinng a copy of your company's businness license or registration certificate.
- For students, need to bring a copy of your student ID or admission notice.
- For those coming to China only as accompanying family members, need to provide a copy of family members business license.
- Registration form of temporary residence form & photocopy
Keep the stomach empty for minium 5 hrs before examination ( Simple water is fine)

Transportation:

Subway Line 10: get off on Shanghai Zoo station(上海动物园）. 15mins far away from the station by walking
Subway Line 2: get off on Bei xin Jing station(北新泾站）,then take the No.196 Bus (西环路 Xi Huan Rd.direction), then get off on Ha mi rd.& qing xi rd.(哈密路&青溪路)

Cross the bridge when you see the this building in Red on Jin Bang Rd.

The entrance of reception just at right side of door gate

This glass house is opposite to the reception building where can provide photocopy & delivery service
Attention:
- Payment: Only accept cash of RMB
- Certification Collecting:
-- In person : after 4 working days, Collect with passport & receipt of examination.
-- By courier : The certificate will be sent in 4 working days , need to fill out the delivery form in person in the glass house and pay the courier fee in advance.
