Japanese Restaurants (Tokyo)

I am in the process of putting together a guide to Protected content the best Japanese restaurants in Tokyo, and was wondering if you were able to recommend any of your favourites.
The parameters I want them to comply with are as follows;
Must be authentic Japanese style restaurant / cafe / bar
Must have English language menu and / or staff available
Should be close to the major metro stations which tourists and business people would tend to be using.
Should be good value

If you are able to send me details including the name, address and website (if any) as well as an idea of the opening hours if you know them.

I am looking to compile them into a smartphone app, so for the first person to recommend each restaurant that I use in the app I will send you a free copy of the app once completed.
