Why is the healthcare system so inefficient in the US?

A couple of months ago I went to an emergency room in Boston. Before I was able to have a meaningful conversation with a doctor, I had to wait for 2.5 hours in the lobby, talk to 4 low-rank staff members, then talk to some intern doctor, who eventually called the real doctor. To crown it all, the valet parking at the hospital turned out to be not free as I had expected, so I had to pay 9 dollars for parking. Is this normal for the US or was it an exception rather than the rule?
