Heart Failure treatment in Europe

Hi there, everyone. My husband and I are considering moving our family to France or Germany for a few years to give our kids some experience living abroad. Unfortunately, I suffer from Heart Failure, and although it's well-controlled with meds, I do need regular follow-up visits. Can anyone share any experiences dealing with chronic illness abroad?
